Patient Information

OFFICE HOURS

The office is open from 9am to 5pm, Monday to Thursday.

Important Preparations before the assessment date:

  • Prior to their appointment new patients will receive some paper work. Please fill out all of the questionnaires and relevant paper work. Please feel free to ask for help from your friends, family or your referrer to complete the questionnaire if required.
  • You may not be assessed if the questionnaire is not completed prior to the appointment.
  • You will receive a phone call or a Short Message Service to confirm your attendance prior to the appointment. If you don't hear from us, please call prior to the assessment date to confirm the appointment.
  • Prepare some brief notes for the common questions below
  • IF for any reason you cannot attend the appointment, please contact Dr Tran's office by phone at least more than 48 hours prior to the appointment otherwise a cancellation fee will be charged
  • Please bring a long all of your investigation results, reports, claim numbers or any other relevant information that you think will be useful for the assessment.
  • Please take (do not skip) all of your required medication(s) on the day.

Common Questions:

Dr Tran will conduct an active history during the assessment, to obtain all the necessary information in order to address the specific questions from your referrer. He prefers to conduct the assessment in simple and basic English language (not medical English). Please prepare some brief dot-point note to these common questions. Preparation of such simple brief answers to the questions below will also be very helpful for Dr Tran's understanding of your problem(s).
  • Do you remember the date when the injury occurred?
  • If you believe the injury is progressive from years of working, could you please let me know the exact date when the symptom became unbearable than usual? When did the symptoms first started?
  • Can you describe what happened that resulted in this injury?
  • Has there been any other injury after this injury date?
  • Could you please make a list of doctors, physiotherapist, any other therapist, or specialist you have seen for the treatment (not assessment for medico-legal purpose) of this injury since it occurred?
Please bring all of the notes you prepared for the above questions to the consultation.

Fees and Accounts

  • This practice charges AMA rates. Private patients with an accepted claim will have their accounts paid by the relevant insurer or self insurer.
  • Pension Concession Card holders will be bulk billed for specific services.

Appointments

  • Patients running late or unable to attend an appointment must call the practice as a lack of appropriate notification may incur a cancellation fee.
  • New patient appointments are scheduled for around an hour of consultation time.
  • This practice aims to be on time for all patients’ appointments, however, occasional unavoidable delays can occur. The receptionist will advise you upon arrival if the doctor has been delayed.

Cancelled Appointments

If you are unable to keep your appointment, please let us know as soon as possible so that we can make the appointment available to someone else. A fee will be incurred for non-attendance without notice of less than 48 hours.
